Absolutely. I could and maybe should have gone into more detail on the good ways to fail (early, by trying risky things, in small ways) which help avoid bigger and more costly failures down the line. And of course, these things are also very context specific. I am coming from the design/high tech context, where we can embrace failures and screw ups as a part of doing business. My sister who is an ER physician thankfully has a different approach. Understanding the cost of various types of failures and putting the right safety nets in place to allow for learning within those parameters is the job of management.
